Information for New Clients:
New clients are required to complete their intake paperwork prior to their first session. To access this paperwork you will receive a link in your email after making your first appointment. If you don't see the email (check your spam folder) please call our office and we will resend it (801) 876-0036. This email will take you to our client portal where you can sign up for an account and complete and sign your intake forms. A link to our client portal is also available above. (You will need to schedule your first appointment with our office manager in order to be able to access this client portal.)
All of our therapy sessions are currently being held ONLINE to avoid the spread and transission of COVID-19. These sessions are conducted using a HIPAA Secure Telehealth software called Doxy.me. You do not need an account to use doxy.me. Simply click this link at the time of your appt to connect:
Fees & Payment Information:
All clients are required to keep a credit or debit card on file with us. This card will be charged automaically in our system at the time of service.
We require 24 hours notice for all cancellations. If cancellation is not received during this time a $60 late cancellation fee will be billed.
We are in Network with:
If we are not in network with your provider we can provide you with a Superbill that you can submit to your provider to receive reimbursement for "out of network" benefits. Please contact your provider ahead of time to verify with them what your "outpatient mental health benefits" are.
We also accept payment from Ecclesiastical Leaders and Wards.
